Enrolling at ACAA

Enrollment at ACAA is open to all residents of Arizona. All students planning to attend ACAA will need to provide the following documents to enroll:

Proof of Residency in the state of Arizona
One of the following:
     -Valid Arizona driver’s license, Arizona Identification card or motor vehicle registration
     -Valid U.S. Passport
     -Real estate deed or mortgage documents
     -Property tax bill
     -Residential lease or rental agreement
     -Water, electric, gas, cable or phone bill
     -Bank or credit card statement
     -W-2 wage statement
     -Payroll stub

The following documents also are required:
     -ACAA Enrollment Packet
     -Withdrawal form from previous school
     -Original birth certificate (copies not accepted)
     -Immunizations Records
     -High School transcripts (high school students only)
     -Middle School Report card (middle school students only)
     -Elementary School Report card (elementary school students only)
     -All special education records (if applicable)
     -Legal guardian or custody papers (if applicable)

We are now accepting enrollments for the 2016-2017 school year. To begin the enrollment process, please select the appropriate grade level packet below and print it out.Once you have filled it out completely, please return it to the school’s front office as soon as possible.
